Important Note: You cannot add HTML forms to a regular TeacherWeb®
page. An HTML form usually includes a command button with "GO" or "Submit" or
some similar command on it. If you need to add a form to your web, you will
need to create the whole web page including the form and add that web page as
an HTMLPage or as a MyPage.
