Welcome to Moody Middle School. Student registration will take place on Tuesdays from 9-2 in June and July. Listed below are the items you will need to bring with you when you enroll your child in our school:
1-Registration Forms (available in the school office)
2-Original Immunization Certificate (blue card)
3-Copy of Birth Certificate
4-Copy of Social Security Card
5-Proof of Residence (utility bill)
6-Proof of Grade Placement (report card or copy of transcript)
7-Custodial Parent or Legal Guardianship (a-Court Document signed by a judge b-Cannot be a statemtent signed by a Notary Public).
