Piano Lessons

 

Students will be instructed in piano from a concert pianist and teacher. The students will be responsible to purchase a music book for this class, you purchase one from any music store.

 

   

Games and Strategy Club

 

 

Bowling at Fiesta Lanes

Children will be picked up by Fiesta Lanes Bowling and taken to the bowling center. A slice of pizza and a drink are included in the price. Parents must pick up their children from the bowling center between 5:00-5:30 p.m.

Please remember there are no clubs on Thursday's. All students that are not attending Bowling must be picked up by 3pm at the latest, as there will be NO supervision for students past this time.

ALL STUDENTS MUST BE SIGNED UP FOR AFTER SCHOOL CLUBS AND FEES MUST BE PAID IN FULL. THERE WILL BE NO CREDIT ISSUED FOR ANY CLUBS MISSED. IN ORDER FOR
THE STUDENT TO REMAIN AT THE SCHOOL AFTER 3:00 P.M THEY WILL NEED TO BE SINGED UP FOR A CLUB. YOUR CHILD WILL AUTOMATICALLY BE SIGNED UP FOR A CLUB AND YOU WILL RECEIVE AN INVOICE IF THE CHILD IS NOT PICKED UP BY 3PM.

EMERGENCY RELEASE
I recognize the risk of illness and injury inherent in any program and am participating in the
express agreement and understanding that I am hereby waiving and releasing the instructors and
Lifelong Learning Academy from and against all claims, costs, liabilities, expenses, or
judgments, including attorney's fees and court costs arising out of participation in the
program. I hereby agree to the aforementioned statement and release Lifelong Learning Academy
and its associates of any financial and/or medical obligation which might be incurred. The
following signature authorizes the Lifelong Learning Academy to pursue any emergency measures
for my child and it is my agreement to pay all costs of collections. This signature also allows
pictures to be taken of my child(ren) for future publication.
