IMMUNIZATION INFORMATION
Varicella (Chicken Pox) in now needed for all 7th and 8th graders unless 
proof of the disease is documented on a State form or on the child's 
immunization card.  The Phoenix Fire Department and the AZ County Health 
Dept. provides shots FREE for children with no medical insurance coverage.
