what materials should I gather for registration?
Two proofs of residency - gas, electric, or water (not phone or cable bill). Birth Certificate Immunization record. Check with your child's doctor to make sure all the immunizations are up to date. Oral Health Requirement: If your child has not had a dental checkup within the past 12 months, make an appointment. You can pick up the Oral Health Form from the school office to take with you to this checkup.Back to Top
