Middle school registration can be completed at the Etiwanda Intermediate School site starting July 27, 2011. It is not necessary to bring your child to register.
At the time of registration, we will need the following:
Birth Certificate (Certified)
Proof of residence (verification of address)
Home owners or renters must submit two (2) proofs of address:
Billings addressed to you at your residence address within the last month from Southern California Gas Company, Southern California Edison Company, Telephone Company (i.e., GTE, MCI) OR any of the following documents bearing your residence address: Homeowners/renters insurance policy, certified copy of escrow papers, current tax return. If you are living with someone else, a notarized form must be submitted along with two bills in the name of the homeowner. It is available with the registration packet.
For homes being built or being purchased:
An escrow document showing the closing date which is scheduled within 30 days of enrolling.
Emergency contact information (the name of local doctor and dentist who will accept your child for emergency care)
Complete immunization record (or a letter from a physician indicating sequence of shots in process). Records and/or dates of the immunizations listed below:
Up-to-date immunization record-original
Report cards, test scores, GATE verification letter, special education records are helpful in placing your student in correct classes.
