REGISTRATION REQUIREMENTS

STUDENT MUST BE RESIDING IN THE DISTRICT AND AVAILABLE FOR ANY NECESSARY TESTING AT THE TIME OF REGISTRATON

Proof of Residence (one form of identification from each list)

 PG&E Bill *  Driver’s License with current address
 Affidavit of Residence  Bank statement or preprinted check
   Car registration or insurance
   Payroll check

*Original current, Pacific Gas & Electric bill. (Photocopies cannot be accepted.) PG&E bills with multiplenames will require two forms of additional proof of residence from List B. If you are a NEW resident, a validrental/lease/sales agreement is required until you receive your PG&E bill. You will be given a conditional registration form for 30 days until a PG&E bill is provided.

An Affidavit of Parent Residence signed by you and the primary resident, must be completed in the Student Assignment Office if you are residing with another person. The primary resident will be required to provide aPG&E bill in their name to verify residence. You will need to provide two forms of identification from List B.

Proof of Age

(Required for kindergarten or first grade entry and students coming from another country)

Certificate of birth (certificates provided by the hospital are not acceptable), a baptismal certificate dulyattested, a passport or an affidavit signed by the parent, guardian or custodian of the child.

Students entering from another country are required to have a passport. 

Immunization Requirements – immunization records are required at the time of registration.

   

•OPV Polio (OPV and/or IPV) - 4 doses

•DTP (Diphtheria, tetanus and pertussis) - 5 doses

or any combination of DPT, DTaP, HDPT or DT

Age 7 years and older (Pertussis is not required)

•Td, DT, or DTP, DTaP or any combination of these - 4 doses

•MMR Measles, Mumps, Rubella (Grades K, 7 and 8) - 2 doses given separately or together

Required for entry into grades 1-6, 8. - 1 dose given separately or together

Age 7 years and older (Mumps is not required)

•HepB Hepatitis B (Grades K, 7 and 8) - 3 doses

•Varicella (Chickenpox) - 1 dose or proof of the disease

•TST MANTOUX TEST- 1 dose

(TB TINE TEST IS NOT ACCEPTABLE)

•The TST must be given within eighteen months prior to entry into kindergarten or six months prior to entry into grades 1–8 from Santa Clara County.
