New Student Registration for Fall 2008

FALL REGISTRATION FOR THE 2008-2009 SCHOOL YEAR WILL BE HELD 

DATES:	    JULY 21 through AUGUST 6, 2008

TIME:		8:30 am to 1:30 pm (Monday through Friday)

LOCATION: Collins Elementary School, 10300 N. Blaney Ave., Cupertino, CA 

PICK UP your Registration packets 7:30 - 4:30 in the Student Assignment
Office, Cupertino Union School District, 10301 Vista Drive, Cupertino
beginning July 9, 2008.   

Beginning on July 21, 2008, registration packets must be picked up at Collins
Elementary School. 

All parents registering their child(ren) must provide: 

•	Kindergarten and first grade only: 
•	Verification of your child’s date of birth in the form of an original
certificate of birth (certificates of birth issued by the hospital are not
acceptable), a baptismal certificate duly attested, or a passport.  

•	All grades:  Proof of residence.  Detailed proof of residency requirements
can be found on the District website under “New Student Registration.”  If you
do not have the required proof of residence, you should contact the Student
Assignment Office – (408) 252-3000, extension 110, prior to registration.  

•	A passport is required of all students entering from outside the United
States.  

•	Verification, in English, is required showing your child’s immunizations are
complete or in the process of being completed.
