Year End - Takin' Care of Business

Parents and Students - Before school ends, please take care of the following:  


CAFETERIA:  
All Students:  All cafeteria balances must be paid off by Thursday, June 5th.
 8th graders -  Please use up your account balances as these can not be
transferred to Lynbrook.  For students staying at Miller, balances will be
carried over for next year.  Students leaving may also have their balances
transferred to siblings.

Lists will be posted on the cafeteria doors and office windows.  Please pay
the cafeteria or the office directly.  Students not paying up their cafeteria
balances, or neglect to turn in late library books or classroom textbooks may
be held back from attending Great America.

NURSE’S NOTES:  Medications in the Health Office - Janice Leier, RN,MS,CNS

If you have stored your child’s medications in the school health office,
please make arrangements to take them home on the last day of school. Any
medication that remains after the close of school will be discarded. If your
student is attending summer school and will need the medication during the
shortened day, please take it to that school site’s main office on the first
day of classes.

If it will be necessary for your child to receive medication during the school
day next year, please pick up a blue Medication Authorization form at the
school office before the last day of school, June 12th. 

In order for either prescription or over-the-counter medications to be given
at school, both the doctor and the parent must sign the form. When completed,
bring the Medication Administration form and the medication (in the original
container) to the secretaries in the school office.

UPDATE YOUR STUDENT'S EMERGENCY CARD

If there have been any changes to your address, home or cell telephone
numbers, contact people, or in your child’s medical condition since the school
year began, please report these changes to the school office so that we may
update your child’s Emergency Card.

Please remember that your child’s Emergency Card must list the names of any
adults who are approved to pick up your child from school. In the event of an
emergency, we will use this information to contact a parent or other
authorized adult.

We will only release students to those adults listed on the card. If you have
 any questions, you may contact us at (408)252-3755.

END OF THE YEAR ATTENDANCE POLICY

Students must be in attendance on the last day of school to guarantee their
place in the district for the following school year. When the student returns,
if space is available at his or her school, the student will be reinstated. If
no space is available, the student will be assigned to another school in the
district where space is available. If the student is ill, parents must notify the
school of the absence and provide documentation.

When you are making travel plans, please consider the possible ramifications.
