What does the "extra" part of the report have to be?

Literally anything you want. Keep it simple, make it fun and let it be kid 
created.  Let your imagination run wild.  You will present whatever you 
choose in a short 1-2 minute presentation to the class. The idea list given 
to you is just a reference for ideas.

Do I need a title page?

Yes, your title page should have your full name, number and title of book.  
NO REPORT COVERS, PLEASE!
