When viewing this document, please keep in mind that the header is 1/2 inch from the top of the page. I'm not sure why it does not appear to be after I uploaded onto my website. Since it is correctly constructed, I don't know how to make it look correct here. Maybe it will print correctly when you print it out. Anyway, the document is correctly created, but you have to use your imagination regarding the top margin.
