PHYSICALS!!!!! You must have a physical to play basketball.
You may use your own doctor or Dr. Flores at the Ontario
Testing center. He especially works with Chaffey students. No appointment is
necessary Monday through Friday 9-5pm address 402 s. Milliken ave, suite E-
2, Ontario Ca. 909-937-3955 Directions: corner of Bricknell & Milliken,
south of the 10 freeway.
INSURANCE!!! You must have health insurance to play.Insurance is your
family's health insurance. You may purchase
insurance through a carrier at school if you need to.
