Please attach the following items to the "Registration Form":

• Copy of latest Report Card -- Grades K-8
• Copy of latest achievement test scores -- Grades 1-8
• Copy of Birth Certificate
• Copy of Baptismal Certificate
• Original (Blue) "Florida Certification of Immunization Form DH 680"
• Original (Yellow) "Health Examination Form HRS-H 3040"
• "School Reference Form" -- Grades 1-8
• Application Fee
• School Copy of Contract

Directions for Application:

1. Complete all information requested on the Registration Form. Please PRINT or TYPE.
2. Obtain a copy of the latest Report Card from the present school for students entering Grades 1-8. Submit a copy of the latest "Progress Report" provided by nursery schools or day care centers for students entering Preschool-Kindergarten, if applicable.
3. Obtain a copy of the latest achievement test scores for students entering Grades 1-8 from the present school and attach to the Registration Form. All students entering Preschool-Grade 1 will be administered the "Brigance Screen." Students entering Grades 2-8 will be administered the "Kaufman Achievement Test."
4. "School Reference Form" to be completed by current or previous school attended for Grades 1-8.
5. A registration fee of $225.00 must be included with the Registration Form. This fee is non-refundable.
6. The application will be reviewed by the administrators of Grace Lutheran School for final decision of admission. You will be notified of the decision once all paperwork has been received and reviewed.

GOOD NEWS SCHOLARSHIPS

     Grace Lutheran School is greatly honored that you have chosen to place your child(ren) with us. We appreciate your trust and recognize that your enthusiasm for our school is very helpful when recruiting new families.

     Currently enrolled members:  Thank you for recommending a new family.  Upon their enrollment, present the "Good News Scholarship Currently Enrolled" card, and you will be credited $500.00 on your tuition ($50.00 per month for 10 months.)

     Newly enrolled members:  Upon enrolling your children in Grace Lutheran School, just present the "Good News Scholarship New Family" card along with your enrollment application, and a $100.00 credit will be applied to your tuition.

 "Good News Scholarship " cards are available in both school offices.
