What needs to be on my lunch check envelope? Who do I make the check out to?

We are getting our lunches from Lakeview Elementary School this year.  Make 
your checks out to Lakeview.  Always send checks and/ or money to school in 
an envelope.  Label the envelope with the student's name, lunch number, and 
teacher's name.
