Packing the Box

PACKING THE BOX –from EXCEPTIONAL-NEEDS@yahoogroups.com

 

___  Put your candidate ID barcode and certificate number label on the side of your portfolio box.
 
___  Put your candidate ID barcode on the cover sheets for each entry

___  Put your candidate ID barcode on each envelope provided for entries and forms


___  Complete and make copies of Contextual Information Sheet(1 for each entry and equal amount of copies)

___  Your candidate ID is in the upper right corner of all commentary pages, artifacts, and verification forms.

___  Your commentary is numbered according to directions.
 
___  Your commentary fits the required format (spacing, font, page limits, etc.)
 
___  Made your classroom layout, made copies, and put in the correct entries.
 
___  Made an enlargement of your photo ID (200%), put on the correct form, made copies, and put in the correct entries.
 
___  Look at each envelope provided for the entries.  If appropriate,  check the your designated path (i.e. vocal,
instrumental, etc.)

___  Written or placed your candidate ID number on all forms requiring it.

___  Look at the Candidate Final Inventory for each entry and make sure you have everything needed for the entry.
 
___  Include the title of the entry in the header of your Written Commentary (along with your candidate ID) on the left side)
 
___  Reread each of your Written Commentaries to be certain that you have answered all the questions in the
instructions.
 
___  Check any Student Work samples against the description you have provided in your written commentary for each student work entry.  Did you refer to the correct student?
 
___  Be sure to make Student Work samples with student identifiers such as "Student A" and "Student B" and cross out student last names.
 
___  View your videotapes to be sure they are audible and viewable, and rewind them prior to submission.
 
___ Make a file (in your room or at home) for your Student and Adult Release forms.  Keep them for at least 3 years (do not submit these with your portfolio)
 
___ Did you run the spell-check program?
 
___ Put all of your Teacher Release Forms together
 
___ Put all of your Final Inventory Forms together
 
___ Put the NBPTS address label on the top of your portfolio box
 
___ For entry 4, the first line is "Description of Accomplishment #__"  or "Documentation of Accomplishment #___".
 
___ For entry 4,  your Descriptions of Accomplishments and Documentation of Accomplishments are sequentially numbered (do not number cover sheets)
