Individual Behavior Plan
The rules are listed below:
1. Talk at appropriate times
2. Do not get out of seat without permission
3. Do not talk back or be disrespectful to an adult or peer
4. Always listen and follow directions
5. Stay on task
6. Do not hit or bother another child/adult
7. Follow rules in the hallways, bathrooms, and cafeteria
8. Participate in all educational activities
9. Do not get into the business of others
POSITIVE CONSEQUENCES
Positive behavior has its own rewards....self respect, the respect of
others, and power through knowledge.
NEGATIVE CONSEQUENCES PLEASE SIGN WEEKLY BEHAVIOR LOG
1 Check - Warning
2 Checks - Verbal discussion w/teacher
3 Checks – Loss of a privilege
4 Checks - Note home to be signed *
5 Checks - Office
*Note - more than 2 4s per week will result in a trip to office
A BEHAVIOR FOLDER WILL PART OF THE T.U.R.T.L.E. BINDER. THE DAILY BEHAVIOR
LOG CAN BE FOUND IN THAT FOLDER. SIGN AND RETURN DAILY! DO NOT REMOVE THIS
LOG OR FOLDER FROM THE T.U.R.T.L.E. BINDER. THANK YOU!!
