REGISTRATION

The following information is required for registration.

JF5 - Registration Form

All the required paperwork must be submitted at the time of registration.

Registration will not be complete and class placement

will not occur until paperwork is complete.

 

Certificate of Immunization (Form 3231)

 

 One of the following boxes should be checked on form 3231:  

“Date of Expiration” or “Complete for School Attendance.”

Note:  If your child’s birthday is between April 1 and September 1, please check the expiration date on your form.  You are responsible for providing an updated form, when your child’s form expires, to the East Side front office.  All students attending a Georgia school must have a current immunization form on file.

 

Certificate of Ear, Eye, and Dental Examinations (Form 3300)

 

This form must be dated August 11, 2008 to present date.  All three boxes (Eye-Vision, Ear-Hearing and Dental) must be checked and signed.  You may have one form for hearing and vision and another form for dental.

 

Birth Certificate

A certified copy of the birth certificate is required.

Social Security Number

 

We will make a copy of the student’s official Social Security card.  Please let us know if you do not want to use the child’s social security number.  Cobb County will assign a waiver number.

 

Proof of Residence in the East Side District

 

Two documents proving residency:  home ownership title, lease or rental agreement, current utility bill, current bank or credit union statement; personal mail delivered by the United States Postal Service; a Cobb County property tax statement; voter registration documentation from Cobb County; a current motor vehicle registration (tag receipt); or Statement of Legal Residence form.

Note:  If you use the Statement of Legal Residence form, you are responsible for having it notarized.  This form has to be renewed annually.
