2008-2009 REGISTRATION REQUIREMENTS
• Immunization Form
-Georgia Department of Human Resources, Form 3231
-Signed by your licensed Georgia Physician or the local health
department
• Certificate of Vision, Hearing, and Dental Screening
-Georgia Department of Human Resources, Form 3300
-Signed by your licensed Georgia Physician or the local health
department
• Official Birth Certificate
-issued from the State or Country in which your child was born. NOT
commemorative certificate from the Hospital. Georgia Law states
that children
must be 5 on or before September 1st to enroll in kindergarten.
• Child’s Original Social Security Card
• Academic/Special Needs Records, if applicable
• Proof of Residency.
To enroll at Sarah Smith, the student’s parent or legal guardian must
be a
resident in the attendance zone of the school. Appropriate
verification must be
your most recent Georgia Power Bill (original). In addition, one of
the following
original documents must be provided.
-Home Mortgage Closing Statement
-Apartment/Condo Lease stating name of guardian and child living
at stated address
-Homeowner’s Insurance Statement
-Copy of Sales Contract (if house in district is under construction
or being purchased/renovated)
