NOTICE TO PARENTS AND GUARDIANS
This booklet contains information that will assist you in registering your
child for public school bus services for the 2008-2009 school year. After
reading the information in this booklet carefully and you decide to purchas
a bus pass, simply tar out and complete the school bus application form on
Pages A1-A2. Submit your completed application form to your child's bus
driver or school office as soon as possible BUT CERTAINLY NO LATER THAN JUNE
20, 2008.
We encourage you to carefully consider your child's school transportation
plans for 2008-2009 before submitted a bus pass application form. The DOE
projects its annual school bussing requirements on the TOTAL NUMBER OF
STUDENTS WHO REGISTER FOR BUS SERVICE. Your application will count toward
the total number of bus seats we contract for your area.
ELIGIBILITY
Pursuant to Hawaii Administrative Rule (HAR) 8-27-5, students must reside A
MILE OR MORE FROM THE SCHOOL WITHIN THEIR ATTENDANCE AREA to be eligible for
regular school bus service.
Students who do not qualify for regular bus service because they live less
than a mile from school, attend a school outside of their attendance area,
or require bus service to and from a location other than home may be
approved for regular bus service but ONLY IF THERE ARE UNUSED SEATS ON THE
BUS AND THAT ACCOMMODATION WILL NOT RESULT IN ADDITIONAL COST TO THE STATE.
Parents can apply for space available bus service by completing a Request
for Student to Ride School Bus on a Space Available Basis, (Form CS-101,
printed on the previous page or available at the school office) along with
the bus pass application form on pages A1-A2 of this booklet. Submit both
forms to the school office for review.
Approval/disapproval for space available bus service requests can take four
or more weeks after the first day of school. No student will be permitted
to ride the bus on a space available basis without prior approval, so
parents are strongly urged to plan accordingly. Furthermore, please note
that space available bus riders are not eligible for free bus service.
PROGRAM FEES AND PAYMENT PLANS
All bus riders (except those who are approgved for free bus service) must
pay the transportation fee. We offer three payment-plan options:
1. Annual Payment Plan - one-time payment due no lather than July 25, 2008.
2. Quarterly Payment Plan - four payments; one for each academic quarter.
a. First quarter payment due no later than July 29, 2009.
b. Second quarter payment due no later than October 9, 2009.
c. Third quarter payment due no later than January 4, 2010.
d. Fourth quarter payment due no later than March 19, 2010.
3. One-Way Bus Coupons - may be purchased at any time.
A complete list of bus pass prices by service type and paymnet plan can be
found on Page A-2 of this booklet (see Part 3 of the bus pass application
form). ANNUAL payment plan prices include a nominal discount.
Students who intend to use the public school bus system as their primary
means of getting to and/or from school daily THROUGHOUT THE ENTIRE SCHOOL
YEAR are encouraged to select the ANNUAL payment plan.
Students who intend to use the public school bus system as their primary
means of getting to and/or from school daily BUT WHOSE TRANSPORTATION NEEDS
MIGHT CHANGE FROM ACADEMIC QUARTER TO ACADEMIC QUARTER are encouraged to
select the QUARTERLY payment plan.
Students who intend to use the public school bus system ONLY OCCASIONALLY
are encouarged to purchase a supply of ONE-WAY BUS COUPONS (additional one-
way coupons can be purchased at any time).
PLEASE SELECT YOUR PAYMENT PLAN CAREFULLY! Although you will be permitted
to change your plan when needed, YOU WILL NOT BE GIVEN A REFUND ON THE
UNUSED PORTION OF ANY BUS PASS unless your child meets one or more of the
creiteria listed in the "Refund Policy" section below.
PAYMENTS
Please do not attach your payment to the application form. Bus pass
paymnets can only be made after your application is approved. You can cehck
on the status of your child's application form by visiting our School Bus
Transportation website at http://ipotal.k12.hi.us/SBT/home.aspx or by
calling our school office. Once your application is approved, please remit
your payment either in person or by mail (please do not mail
in cash payments). All payments are due prior to the first day that your
child intends to use the bus service.
Please call the school office for the dates and times that in-person bus
pass payments are being accepted or for instructions on how to mail in a
payment. Payments must be made by cash, check, money order or cashier's
check payable to "Department of Education".
RETURNED CHECKS
There is a $25 service fee for all returned checks. The service fee (a
separate payment made payable to "Director of Finance") AND the balance owed
on yoru child's bus pass account (made payable to "Department of Education")
must be paid in person at the school office by CASH, MONEY ORDER OR
CASHIER'S CHECK within one week of the returned check. Failure to remit
both payments by the deadline could result in the immediate suspension of
bus riding privileges for your child.
FREE BUS PASS
Students who meet one or more of the following criteria are eligible to
receive a free bus pass:
* Student qualifies for FREE lunch (Note: Students who qualify for reduced
lunch prices are NOT eligible for free bus pass).
* Student is a foster child.
* Student is *homeless (McKinney-Vento Act).
* Student's Special Education IEP/MP includes regular transportation as a
related service.
* Student is directed by the District to attend a school outside of the
designated attendance area.
* Student has three older siblings who are fare-paying us riders.
Please be sure to complete Part 2 of the school bus application form if you
believe your child is eligible for a free bus pass, making certain to check
all of the criteria categories that apply.
REFUND POLICY
Only students who meet one or more of the following criteria are eligible
for bus pass refunds:
* Transferred to another school.
* Moved to a residence located within a mile of school
* Qualified for free lunch after purchasing a bus pass
* Became a foster child after purchasing a bus pass.
* Became *homeless (McKinney-Vento Act) after purchadsing a bus pass.
* Directed by the District to attend a school outside of the designated
attendance area after purchasing a bus pass.
* Three older siblings purchased bus passes after youngest sibling
purchased a bus pass.
To apply for a bus pass refund, you must request a refund at the school
office together with your child's bus pass card. Your final refund amount
will be based on specific factors surrounding your request including account
status, last day of service and refund eligibility criteria. All approved
refunds will be mailed directly to your residence or forwarding address.
Refunds are processed on Oahu through the Student Transportation Services
Branch.
Once-way bus coupons are non-refundable.
BUS PASSES
Your child will be issued a bus pass ONLY UPON SATISFACTORY RECEIPT OF A
PAYMENT (STUDENTS WHO QUALIFY FOR FREE BUS SERVICE MAY PICK UP THEIR BUS
PASS UPON AVAILABILITY). Once payment is made, your child will
automatically receive a temporary bus pass which will enable him/her to
start using the school bus service right away. A permanent bus pass card
will be made available for pick up at your child's school office some time
prior to the first day of school (please call the school office to
confirm). Students must surrender their temporary bus pass in order to
receive their permanent bus pass card.
All studens must show a valid Department of Education bus pass to the bus
driver every time they board the school bus. If your child refuses or fails
to display a valid bus pass upon demand, he/she will be reported to the
School Principal and amy be denied a ride.
LOST BUS PASS FEE
If your child loses his/her permanent bus pass card, you will be required to
submit a Request for Duplicate Bus Pass form (ST-73) to your child's school
office along with a $5.00 dubplicate bus pass fee. Your child will be
issued a temporary bus pass upon payment of the fee. The temporary bus pass
will be valid for no more than twenty days while the replacement permanent
bus pass card is printed and delivered to the school office for pick up.
The student must surrender the temporary bus pass in order to receive the
new permanent bus pass card.
SCHOOL TRANSFERS
If your child transfers to another school, you will be required to apply for
a refund at your child's former school and purchase a NEW bus pass at your
child's new school.
BUS STOP AND BUS ROUTE INFORMATION
Information regarding bus routes, bus numbers, bus stop locations and pick
up times can be obtained by calling your respective school bus company. A
complete list of bus company names and contact information can be found on
our website at http://iportal.k12.hi.us/SBT/home.aspx or by calling your
local school office.
