4th Grade Supply List

Ms. Kukurza’s Fourth Grade Supply List

Welcome back!   Your child will need to bring the following items on the first day of school:  Please be sure to label everything with your child’s name.

 

-  Chicago Public Library Card

-  Art smock

-  1 roll of paper towels

-  1 large box of kleenex

-  1 boxes of diaper wipes

-  1 soft pencil case with three-ring binder holes

-  1 three-ring soft cover binder that zips closed!   Please make sure it is at least 2 inches wide. 

-  2 Hard Cover Binders (1 white 1 black : both need to be 2 inches wide)

-  2 packages of “Avery 8-Tab Set, Plastic Dividers with Tab Labels” (these are found at Staples) Please label one set ahead of time with the following labels: Math, Science, Social Studies, Paper, Working with Words, Time for Kids, TTS, and Guided Reading.  Please label the second set with the following words: References, Notes, Assigned Writing, Paper 1, Paper #2, Paper #3, Paper #4, Dedication Pages,

-  Pencils (Mechanical Pencils are fine too.)

-  Ruler

-  Protractor

-  Compass

-  Scissors

-  Glue Stick

-  Colored Pencils

-  Markers

-  Blue erasable pens

-  Red Pens (at least 5)

-  2 vis-à-vis markers

-  3 packages of loose-leaf paper (wide-ruled)

-  6 packages of lined index cards

-  5 two-pocket folders. Please label them ahead of time with the following:         Friday Folder, Computer Folder, Spanish Folder, Ideas, Abandoned        Writing

-  2 Spiral Notebooks (one-subject, wide-ruled)

-  Small or large clear Ziploc plastic bags

-  Stickers

-  Cheap “Friday Raffle Prizes” examples: pencils, paper, bubbles, pens,   small toys, anything from the dollar store, etc…..

-  Skinny Permanent Markers

-  1 Pack of Dry Erase markers

-  2 Cloth book covers

-  Disposable camera

-1 pack of Clear Plastic Paper covers that fit into a binder

-$2 towards science experiment perishable materials

 

·      We are looking for new or gently used books for our classroom library.  If you have any of these books at home that you would like to donate at any point during the year, we would greatly appreciate it.  Ex. Historical fiction, mystery, realistic fiction, adventure, biographies, newspapers, magazines, cookbooks, recipes, brochures, old Hallmark cards, Religious books, informational books, etc.

 

All new students must have a copy of their birth certificate, baptismal certificate (if Catholic), and immunization record.
