Health Requirements:
Students entering Kinnikinnick schools for the first time at any grade level and students in Pre-k, Kindergarten and 6th grades are required to have a physical exam on file with the school nurse before the FIRST DAY OF SCHOOL. The "Certificate of Child Health Examination" must be COMPLETELY filled out. The "Diabetes Risk Assessment and BMI" section MUST be completed by the health care provider and the "Health History" portion MUST be completed and signed by the parent/guardian.
Immunizations must be up to date and complete before the FIRST DAY OF SCHOOL. Incomplete examinations will not be accepted.
Eye Examination Requirements:
Eye examinations are required for all children entering Kindergarten and
children entering any Illinois school for the first time. Completed examination forms are due tothe school nurse by October 15th, 2009. Forms can be downloaded from the "Health Forms" page. If you are unable to meet this requirement, please contact your school nurse.
Dental Examination Requirements:
Dental Examinations are now required for all Kindergarten, 2nd and 6th grade students. Exam forms must be submitted to your school nurse before May 15, 2010. If you are unable to meet this requirement, please contact your school nurse.
