In order to schedule an appointment, please print and complete the first form. If your child is starting kindergarten in Fall 2012, print and complete forms 1-7 and bring them to your appointment. The parents of preschool-age children need to print and complete forms 1,2,3,6,7,& 8 and bring them to the screening.
