Download, complete and save the attached document. Be as detailed as possible. If you are asking someone to
write you a letter or recommendation it is very important that they have all the information about your
accomplishments and what you have done so far. Even if they have known you for all 4 years of school, they
can't possibly know everything about you.
When asking someone to write a letter on you behalf, be curtious and allow them plenty of time. DO NOT ASK
THEM THE DAY BEFORE IT IS DUE! It takes time to write a letter.
Typically ask at least 3 weeks in advance of when you would like it do. Give the teacher a copy of the
Request for Letter of Rec. form as well as an envelope that is already addressed to the school you're sending
it to and stamped.
Make sure to follow up with the teacher 2 weeks later and of course, always thank them for taking the time.
