Mandatory clothing for physical education includes Forest Ridge Academy T-shirt and Forest Ridge Academy shorts and an extra pair of gym shoes to be kept at school. Forest Ridge Academy sweatshirts are strongly recommended for PE for those times students go outdoors for PE.

PLEASE RETURN THIS COMPLETED FORM to Forest Ridge Academy BY AUGUST 1, 2005. This will provide ample time to have all uniforms available for the opening of school.
