Kg Enrollment Policy

To: Parent/Guardian

Welcome to Kindergarten and USD 353!  You are about to start on a new 
journey with your child--school. As your school nurse, I would like 
to assist you in preparing your child for school.

THE FOLLOWING IS A LIST OF REQUIREMENTS AND RECOMMENDATIONS FOR 
KINDERGARTEN ENROLLMENT:
      
        1)PROOF OF CURRENT IMMUNIZATION-REQUIRED
          (4DTP with at least one dose on or after the 4th birthday; 
          4 Polio with at least 4 weeks between each dose/or/3 Polio 
          with at least 4 weeks between doses & one dose on or after 
          the 4th birthday; and 2 MMR's, Hepaitis B (3 shot) and one dose
          of varicella (chicken pox) vaccine or disease.

        2)PHYSICAL-REQUIRED

        3)Vision check up-Recommended

        4)Dental check up-Recommended

KANSAS LAW REQUIRES CHILDREN TO GET A PHYSICAL AND BE CURRENT ON 
IMMUNIZATION TO BE IN SCHOOL.  PLEASE HAVE BOTH DONE BY THE FIRST DAY 
OF SCHOOL. USD 353 DOES HAVE AN EXCLUSION POLICY. THIS MEANS ANY 
CHILD WHO DOES NOT HAVE A PHYSICAL AND/OR CURRENT IMMUNIZATIONS WILL 
NOT BE ALLOWED IN SCHOOL UNTIL DOCUMENTATION HAS BEEN RECEIVED BY THE 
SCHOOL THAT THIS HAS BEEN DONE.

Your child's doctor may complete the physical & immunizations.  
Physicals and immunizations are also avaible through Sumner County 
Health Department.   They have Registered Nurses who can give your 
child their physical and/or immunizations.  There is a fee for both 
and they do have a sliding fee scale.  You must bring proof of 
income.  No child is turned away due to inability to pay for his/her 
immunizations.  Please call 326-2774 for an appointment.

Eye specialists recommend all children to have an eye exam before 
school entry at 5-6 years of age.  In Kansas, we have the See To 
Learn program.  Free eye assessments are given to children at 3 years 
of age.  Information on the See To Learn program is available in my 
office. If your child has vision problems, or is already seeing an 
eye doctor, follow his or her recommendations on frequency of eye 
exams.

It is recommended all children have yearly dental check-ups. School 
districts are required to have a dental program.  USD 353 uses dental 
cards for our program.  One is enclosed in the kindergarten packet. 
You can also ask your dentist to provide you with a written note 
regarding your child's check up.
