6th- 12th Grade Student Immunization Requirements
for 2009-2010 School Year

4 DTP, Dtap and or TDAP – Usually given prior to Kindergarten enrollment –
Tetanus diphtheria pertussis booster Requirements are as follows:
Tdap (Tetanus, diphtheria, pertussis) – Is required for any student
who 11 years of age on 8/31/2009.
Exisiting Tetanus Booster Requirement- 10 year booster requirement -
A Tdap booster is required for students older than age 11 on
9/1/09. This booster is required 10 years after they have
completed their initial series of Tdap/Td or DTaP/DT
vaccinations.
3-4 OPV or IPV (Polio) - Dates & requirements usually reviewed prior to
entering Kindergarten.
2 – MMR (Measles, Mumps, Rubella) – The first dose must be on or after the
1st Birthday. Dates & requirements usually reviewed prior to entering
Kindergarten.
Hepatits B Vaccination series – Three (3) doses (Required for all students
up through the 9th Grade – Recommended for all students)
Varicella (Chickenpox) – One (1) dose of the vaccine if the student has not
had the Chickenpox disease. (Required for all students up through the
9th Grade – Recommended for all students)
OR -If your student has had the Chickenpox:
By the end of this school year (08/09) please send a signed dated
note with the date your student had the chicken pox.
If no note is received before the start of the 2009-2010 school year
the Kansas State Regulations state that you will be required to
obtain a written note from your doctor to verify the date your
student had the Chicken Pox.
Legal Alternatives to Vaccination Requirements “KSA 72-5209” –
1. "Annual written statement signed by a licensed physician (Medical
Doctor/M.D. or Doctor of Osteopathy/D.O.) stating the physical condition of
the chi to be such that the tests or inoculations would seriously endanger
the life or health of the child." Medical exemption shall be validated
annually by physician completion of KCI Form B & attachment to the KCI.

2. "Written statement signed by one parent or guardian that the child is an
adherent of a religious denomination whose religious teachings are opposed
to such tests or inoculations."
Non-compliance with the provisions of KSA 72-5209 as amended will result in
the child being excluded form school until compliance with the law has
occurred. The building principal or designee (often times the school nurse)
will notify the parent or guardian in writing of the date the child will
be excluded from school. The notice shall indicate the reason for exclusion
from school attendance; state The pupil shall continue to be excluded until
the pupil has complied with the law; and inform the parent/guardian that a
hearing shall be afforded to them upon request. On the fourth consecutive
day of absence due to exclusion the student will be reported to the SRS or
Juvenile Court System as truant.
** Acceptable written proof of immunizations recieved include:
A copy of the student's immunization card from the Health Dept. or their
form documenting that the Td or Tdap shot was given.
A prescription pad or letterhead form from your doctor with the date the
Td or Tdap shot was given, signed by the Physician, Physician
Assistant or Nurse.
