Soccer tryouts Girls - Nov. 14th and 15th 4:00 - 5:30 Boys - Nov. 16th and 17th 4:00 - 5:30 1. The student athlete must have a physical on an LHSAA form. 2. The student athlete must have a parental consent form signed. 3. The student athlete must have a school waiver form signed. 4. The student athlete must have health insurance. 5. The student athlete must maintain a 1.5 GPA with no more than two failures on the previous report card. 6. All forms can be picked up from and returned to Coach Cochran.
