Internship

CAREER INTERNSHIP POLICIES


The student intern and his/her parents/guardians must understand, observe, 
and adhere to the following policies and responsibilities.


--An intern student must be at least sixteen (16) years of age by the first 
day of the 2008-09 school term.  Pharmacy Tech students must be eighteen 
(18) by July.

--All interns under the age of 18 will be required to have a work permit.  I 
will give out the forms needed and issue the work permits once all 
information is received.

--All internships are unpaid.

--An intern student must complete the full term in order to receive credit 
for the internship program.  No intern may drop the course at any time and 
receive partial credit.

--Intern students are expected to attend school each day.  If a student is 
absent from school, he/she MAY NOT report to the training site.  If a 
student excessively misses school and/or work site training, the student 
will be removed from the program and no partial credit will be given.

--A student who cannot attend work site training must notify his/her 
mentor.  THESE CALLS ARE EXTREMELY IMPORTANT AND ARE THE RESPONSIBILITY OF 
EACH STUDENT.

--The intern student should maintain a cumulative and quarterly unweighted 
G.P.A. of at least 2.5 and present NO DISCIPLINE problems.

--The intern student must present a pleasing appearance.  Good grooming and 
hygiene must be at their best.  Since interns leave from school and report 
to their work site, school uniforms are worn.  If school uniforms are not 
worn, interns must adhere to each individual training site’s dress code.  
Allied Health interns will be required to purchase navy blue scrubs 
containing the Allied Health emblem.  These scrubs, as long as they are the 
ones approved by the coordinator and principal, may be worn to school only 
on the days when the student reports to the training site.  If an intern 
comes to school in scrubs on days when he/she does not report to their 
training site, he/she will be in violation of the NHS dress code policy and 
subject to disciplinary action.

--The intern’s shadowing experience must average a minimum of six (6) hours 
a week to receive full credit at the end of the term.  The days and times 
for interning are set by the coordinator and vary from site to site.  All 
shadowing experience must be completed by 4:00 p.m. on school days as per 
school board policy.

--Dishonesty at school or at the training site will not be tolerated.  Any 
student who is caught in a dishonest act is subject to removal from the 
program with a failing grade.  

--All intern students are required to sign a confidentiality agreement.

--Students participating in the Internship program must comply with the 
rules set by Northshore High School, their training site, and their 
supervising mentor(s).

--A student may not, under any circumstance, quit his/her training station 
without prior approval of the coordinator.  Any problems at the training 
site should be discussed with the coordinator to bring about proper remedy.  
Any parent who has a problem with their child’s training site should contact 
the school coordinator.  This program is operated through the school and 
problems should be addressed with the coordinator first.

--The days of the week and hours of the internship experience will vary from 
one training site to another.  There may be days when a student does not 
have to report to their training site.  In this case, the student may 
not “hang around” the NHS campus.  They are to sign out and leave campus.

--Students will report to the coordinator’s room EVERY DAY before they leave 
NHS and sign out.

--Travel to and from the assigned training site is the student’s 
responsibility.  Each student and his/her parent/guardian must sign a 
transportation permission form.

--Weekly time records MUST be turned in each Monday during sign out.  Any 
time record not received on Monday is subject to grade penalty.

--Each intern will be required to show some type of appreciation to his/her 
mentor(s) at Christmas time and at the end of the year.

--Intern students will receive a grade each nine weeks based on sign out, 
weekly time record, and evaluations by the mentor.

INTERNSHIP GRADES



Grades in the internship program will be based on the following:

20%--Daily sign out
20%--Weekly work record
60%--Mentor’s evaluation




Daily sign out

You must sign out on a daily basis.  If you forget to sign out, it is 
counted against you.  If you are absent and athe absence is not excused, it 
is counted against you.  If you are absent, make sure it is an excused 
absence and you get me to sign your blue slip.  If you check out and the 
absence is not excused, it is counted against you.  Always get a blue slip.  
Attendance is extremely important in this program.  Having too many absences 
can remove you from the program.



Weekly time record

The Louisiana State Department of Education requires verification of the 
hours that are spent at your training site.  This form is verification of 
those hours.  Weekly time records will be turned in on Monday, during sign 
out, for the previous work week.  Any time record not received on Monday is 
subject to grade penalty.  Any weekly time record submitted after one week 
will be recorded as a zero.  EMT and CNA students are not required to turn 
in weekly time records.  They must meet the requirements as established by 
LTC.


Mentor evaluation

At the end of each nine weeks, you will be asked to bring an evaluation for 
your mentor to complete.  Your mentor should discuss his/her evaluation with 
you.  This evaluation will be due at the end of the nine weeks.  Make sure 
you pick up the evaluation from me, have your mentor fill it out, and return 
it to me by the due date.  EMT and CNA students’ grade in their LTC class 
will count as their mentor evaluation.

CONFIDENTIALITY AGREEMENT


	
As a participant in the Northshore High School Career Internship Program, I 
understand that I must follow all rules, regulations, and policies of my 
training site.  I further agree to maintain strict job and workplace 
confidentiality during and following my participation in this program.  If I 
breach this agreement, I will be removed from my position and will receive a 
failing grade for this course.

TRANSPORTATION PERMISSION FORM



	I understand that as a participant in the Career Internship Program, 
my child must provide his/her own transportation to and from his/her 
assigned training site.  The car in which my child will be driving is 
properly insured as required by state law and he/she is properly licensed.

	I will hold neither the St. Tammany Parish School Board, the 
Superintendent, Northshore High School, the principal, the school-to-work 
coordinator, the assigned training site, or the mentor(s) responsible for 
any accident or injury to my child that may occur on the drive to/from the 
assigned training site.
