Dear Parent/Guardian:

State law mandates that a physical exam be done in grade one. If a physical
exam has been done and you have not given me a copy, please forward a copy of
the results.  If a physical exam has not been completed, please forward the
results after your appointment. Thank you for your co-operation in this matter.
