We are here to help make the process of moving and starting a new school as
smooth as possible.
Will you be new to the Medfield School District
and Blake Middle School for the 2008-2009 school year?
We will be having two registration dates:
Wednesday, June 10, 2009 at 3:00 PM
Thursday, August 27, 2009 at 10:00 AM
Please call the guidance office at 508-359-2445 and speak with Andrea Moores
to make an appointment to register.

Incoming 7th and 8th graders will need about 2 hours set aside for placement
tests.
Students will be given a tour and will have all their questions answered.
A parent/guardian will have to fill out the necessary paperwork.
PLEASE READ ALL THE INFORMATION BELOW TO KNOW WHAT TO BRING WITH YOU THAT
DAY!
Things you need to have in order to register:
-Copy of Birth Certificate
-Copy of Purchase and Sale or Lease (proof of residency)
-Records Release from Previous School or records (bring name, address, fax #
of previous school attended.)
-Health records. (Please visit the Health section of the web site--
immunizations and an updated physical.)
When you come to our school you will need:
-to take placement test for Math and Reading (7th and 8th grade)
-to tell us what language you will be taking,
French or Spanish (7th and 8th grade)
-let us know if you want to be in band, orchestra or chorus (or none)?
-7th graders will need to get all the Nature's Classroom information
(this can be found on through the Blake home page)
-8th graders will need to get the forms for the swim test
(for the canoe trip in the fall)
