Immunizations
Just a reminder, if your child is given any new shots at his/her doctor's
visit please ask for an updated immunization record and send it in. Then I
will update your child's record.

BMIs
I will be screening children in grades 1 and 4.
Reminder notes to parents of fourth grade students are being sent home.
Please have your student's health care provider complete the physical exam
form and return it to me. I am also asking parents to complete the interval
health history form and return that, also.
