Attach a Box Top to each of the 10 symbols on the contest entry form.
TAPE ONLY – no staples please!
Expiration dates must be valid and clearly visible
Each completed sheet is 1 contest entry.
Make sure your first and last name, grade, and teacher are on your
entry!
Submit your Box Tops.
Place Box Tops in the bin near the office at Taft or turn them in to your
teacher.
All contest collection sheets must be submitted by Wednesday November 19
.
2 winners will be drawn from all entries submitted by the entry deadline.
Each winner will receive a Webkinz Blue Jay.
Additional collection sheets are available near the Taft Box Tops bin, or on
the PTO website.
