To register at the Watertown Public Schools, please bring the following documents:

• A copy of the student's birth certificate (in English)
• Two proofs of residency: These bills must be recent and must have the parent's name on the account
• A copy of gas or electric, PLUS
• A copy of cable bill, lease, or telephone bill
• A copy of the student's medical record (immunization dates)
• A copy of a recent physical (within the past 12 months) with a certificate of vision from the physician
• A COMPLETED packet Registration Packet

IMPORTANT: "Watertown Public Schools Records Release Form I" - please fill in student's previous school name and address in the top section and be sure to sign the bottom of the form
