What do I need in order for my child to have medicine at school?

Your child's physician must complete a "Physician's Authorization of 
Medication For A Student At School" Form for Prescription medications and 
some over the counter medications. An Over-The-Counter Medications Parental 
Permission form for certain medications may be completed by the parent. 
Parent signature is also required on both forms.

What type of container can the medicine be in?

The medicine must come in the bottle from the pharmacy with your child's 
name,medication,dosage, and date of prescriptions;for over-the-counter 
medicine it must be provided in the original unopened container to the 
school.

Who can bring/pick up medications?

The parent/legal guardian must bring in and pick up all medication... the 
student is not allowed to transport medications.

What medications are listed on the Over-the-Counter Parental Permission Form?

Tylenol or generic for pain per package dose.
Advil/Motrin or generic for pain per package dose.
Antacid medication per package dose.
Calamine lotion or 1% anti-itch cream/spray
Sunscreen
Over-the-counter antibiotic ointment
Cough drops/throat lozenges

All of the above Medications will only be administered as needed and 
once/daily;if more frequent times are needed please have your physician 
complete an Authorization for meds at school form.
