At the beginning of the school year, all Student Information Forms and Kindergarten Health Assessment Forms are reviewed in order for the nurse to be aware of any medical/health concerns.

        Please contact me if your child has any medical concerns, chronic diseases, life threatening allergies, etc. that may affect his/her academic, personal, or social performance or his/her safety at school.

         Together we will develop an Emergency Action Plan (EAP) to address appropriate medical interventions to ensure your child's well being at school.
