BOOK REPORT GUIDELINES
1. DUE DATES
a. Topic Due Date on or before September 18, 2008
b. Book Report will be due on October 27, 2008
Second Semester
a. Topic Due date on or before February 19, 2009
b. Book Reports will be due on March 23, 2009
GUIDELINES FOR BOOK REPORT
1. Each student is required to complete two book reports, one each 9 weeks.
2. The topics and reports are due on the assigned dates.
3. The report will be done on a disease, health of fitness issue or a
sports personality .
4. Reports on the history of a sport WILL NOT BE ACCEPTABLE.
5. Duplicate of a report will not be acceptable. Example, two people
cannot do a report on Michael Jordan.
6. A listing of topics will be posted.
Format
a. THE REPORT MUST BE TYPED.
b. The report must be a minimum of 5 typed pages. The title page,
introduction and resource page are not a part of the 5 pages.
3. The standard font of 12 is required and type of letters “Times New Roman.
4. The top and bottom margins are 1”.
5a. The right and left margins are 1.25”.
5b. Report should be double spaced.
6. Bind the report together (binder or staple).
7. Your report should include the following:
a. Cover sheet (should be an illustration of your report)
b. Title Page
c. Introduction
d. Body of the paper (5 typed pages)
e. Conclusion
f. Resources (minimum of 3)

Writing Assignment
Interview/Report
Guidelines
1. Each student is required to complete one interview.
2. The interviewee, interview questions and final report are due on the
assigned dates
3. The interview will be with a person in health care, college or
professional athlete, high school or college coach, wellness or fitness
professional or nutritionist.
4. Only one interview from a particular person.
5. The interview will be written into a 5 pages report.
6. You should include the interviewee’s name, telephone number and e-mail
address.
Due Dates:
September 18 - Person name and interview questions
October 27- Interview Report Due
February 19, 2009- Person name and interview questions
March 23, 2009- Report is Due
Format:
1. All work must be typed
a. Interview questions
b. Interview reports
2. The interview reports must be a minimum of 5 pages (content)
3. The standard font of 12 is required and style of type should be Time
Romans.
4. The top and bottom margin are 1”
5. The right and left margins are 1.25”
6. Report should be DOUBLE SPACED.
7. Bind your report.
8. Your report should include:
a. An illustrative cover sheet
b. Title page
c. Introduction: Why you chose the particular profession.
d. Content: The interview report (5 pages minimum)
e. Conclusion: What you learned from the interviewee.

• Anyone in a health profession
• College or Professional athlete
• Highschool coach, college coach, or professional coach
• Sports commentators
• Sports photographers
• Athletic trainers
• Physical therapists
• Sports Agents
• Athletic Directors
• NOTE: A duplicate report ON the same person is NOT acceptable
