Learning Support Referral Process

Parent or teacher has a concern about studentís performance

1. Teacher and Parent conference. If the teacher feels it is necessary the Learning Support teacher may be asked to attend this conference.

2. Teacher and Parent work together to implement new strategies. (Teacher may request small group intervention by Learning Support Teacher for students in grades K-3.) If public school testing is being considered the classroom teacher documents the use of strategies and their outcomes for a period of four weeks. (Interventions should be documented on an RE2 form.)

3. The Learning Support teacher observes the student in class and completes any necessary screening requested by the teacher and/or parent after parental permission has been obtained. The Brigance Inventory of Basic Skills in among the instruments used. Other information used by the Learning Support Teacher may include previous report cards, IOWA or other standardized test results and other information in the students permanent record. The Guidance Counselor may also assist in this process.

Follow-up Conference

Following the collection of information a meeting of parents, teacher(s), & Learning Support will be held to discuss options based on screening results and implementation of strategies by parents and/or teachers. The Guidance Counselor and Principal may also attend this meeting if needed. A recommendation for consultation with an outside professional may be made at this time.

1. Students must have a current vision and hearing screening report on file with the school. This can be obtained from the student's family physician.

2. For private testing, all of the gathered above information is collected and sent to outside professional/agency.

3. For Forsyth County School System the Guidance Counselor at the student's residential district school is contacted. RE 2 forms are forwarded to appropriate personnel at the residential public school.

Appropriate consent forms provided by the outside agency must be signed in order for information gained from evaluation to be shared with the school.

Documentation of diagnosed disabilities may include reports from a Medical Doctor in the case of ADD/ADHD or other health impairments, a Speech Pathologist and/or Audiologist in the case of Speech and Language or hearing disorders (including CAPD), and/or a licensed psychologist in the case of specific Learning Disabilities or other disorders.

504/Student Accommodation Plan

Once the evaluation is received and reviewed by school personnel an accommodation plan will be put into place. Teachers and parents work as a team to write an appropriate plan for the student that will include accommodations and modifications for instruction and/or testing as well as any Learning Support services to be received. The Principal or Principal Designee must approve the Student Accommodation Plan before it is implemented. The 504/Student Accommodation plan is reviewed annually.
