Remember, if you are planning on trying out for a sports team a physical
must be on file with the nurse before tryouts. Check with the nurse to make
sure you have one, because no one will be allowed to try out without one.
Physicals for sports are only good for one calender year. If your physician
will not give you another physical I need a note from the doctor saying that
it is alright for the child to participate until their next physical.
Forms are available outside the office,the physical form can be downloaded
from this site. If you take your child for a physical it would be a good thing
to send in a copy to the nurse.
This will alleviate the problem of your child/ren not being able to tryout
for sports during the year. This will also help me keep updated immunization
records on your child/ren.
If you wish your child to take any over the counter medication for colds
please send in with a note, and I will be glad to give it to them. Students
are not allowed to carry medications around with them.
Please remember to pick up any medications from the nurse's office before the
end of school.
