Announcements

WE ARE NO LONGER ACCEPTING CONSENT FORMS FOR THE H1N1 VACCINE. ONLY STUDENTS 
WITH A CONSENT FORM ON RECORD WILL BE RECEIVING THE VACCINE.I WILL BE SENDING 
HOME A CONFIRMATION LETTER FOR ALL STUDENTS FROM WHOM I HAVE RECEIVED A 
SIGNED CONSENT. YOU WILL BE NOTIFIED BY GLOBAL CONNECT AS TO THE DATE OF 
SCHOOL ADMINISTRATION. PLEASE CALL MY OFFICE AT 732-613-6828 IF YOU HAVE ANY 
QUESTIONS.





Fifth grade parents please be aware of the immunization requirements that 
need to be met for your child to enter Hammarskjold Middle School in 
September 2010. 
Immunization requirements are given below.

East Brunswick Board of Education is following CDC guidelines in regards to 
fever. Children are to stay home until they are fever free for 24 hours 
without taking fever reducing medication.


IF YOU HAVE ANY CLOTHES THAT YOUR CHILD HAS OUTGROWN 
WOULD YOU PLEASE CONSIDER SENDING THEM IN TO THE NURSE'S OFFICE. OFTEN 
CHILDREN HAVE ACCIDENTS OR SPILLS AND REQUIRE A CHANGE OF CLOTHING. (Pants 
and socks are especially needed.) Also if your child borrows clothing from 
the Nurse's Office please wash and return it so that others may use it. Thank 
you.



 Any student entering Chittick Elementary  must meet the 
current immunization requirements set forth by the New Jersey Department of 
Education. They must also have a current physical exam on record. This 
physical must be done within the past calendar year.

The following documents are required for registration:
4 forms of proof of residency
Original birth certificate with raised seal
Complete immunization record
Proof of a current physical exam (done from September 2008 to September 
2009). Based on September 2009 entry.

The NJ Department of Health and Senior Services are requiring the following 
additional immunizations for preschool children:

Pneumococcal conjugate Vaccine (PCV)
Every child 12 months to 59 months of age enrolled in a preschool on or after 
September 1, 2008 shall have recived at least one dose of PCV on or after 
their first birthday. This must be completed before the child enters school.

Influenza Vaccine
Children 6 through 59 months of age attending preschool on or after September 
1, 2008 shall annually receive at least one dose of influenza vaccine between 
September 1 and December 31 of each year.

 The following immunizations are now required for every child born after 
January 1, 1997 and entering grade 6 in September 2010: 
Diptheria and Tenanus Toxoids and Pertussis Vaccine
Every child born on or after January 1, 1997 and entering Grade 6 or a 
comparable age level special education program on or after September 1, 2008, 
shall have received one dose of Tdap (Tetanus, diphtheria, acellular 
pertussis)given no earlier than the tenth birthday.
Children entering or attending Grade 6 on or after September 1, 2008 who 
received a Td booster less than five years prior to entry shall not be 
required to receive a Tdap dose until five years have elapsed from the last 
DPT/DTap or Td dose.
Children born on or after January 1, 1997, transferring into a NJ school from 
another state or country after September 1, 2008, shall have received one 
dose of Tdap, provided at least five years have elapsed from the last 
documented Td dose.

Meningococcal Vaccine
Every child born on or after January 1, 1997, and entering or attending a 
comparable age level special education program on or after Sepotember 1, 
2008, shall have received one dose of a meningococcal-containing vaccine, 
such as the medically preferred meningococcal conjugate vaccine. The vaccine 
must be given after the eleventh birthday.
Every child born on or after January 1, 1997, and transferring into a NJ 
school from another state or country on or after September 1, 2009, shall 
have received one dose of meningococcal vaccine.

All vaccine doses that are administered less than or equal to fours days 
before either the specified product label minimum age or dose spacing 
interval shall be counted valid and not require revaccination in order to 
enter or remain in school.

Please send a copy of your child's updated immunizations (physician note or 
immunization card) to the school nurse documenting that the above 
immunizations have been given to your child by September 2010.
