Information Regarding Medication Usage


All prescription and over the counter medications shall only be 
given to a student during a school sponsored field trip by either 
the student�s parent/guardian or by a duly authorized registered 
nurse.  In the absence of the parent, the authorized nurse will 
administer all medication on the trip.  Those students who meet the 
criteria for self administration of medication for a potentially 
life threatening condition (ie. asthma and severe anaphylaxis) may 
be in possession of an asthma inhaler or one dose of Benadryl with 
an Epi-pen, provided the proper doctor�s orders and parental 
permission forms are on file in the nurse�s office.  Organizing and 
recording medication orders and packing for a trip of this size 
takes quite a bit of time.  Meeting the deadlines noted below will 
be greatly appreciated. 
If your child will require any prescription or over the counter 
medication while on the overnight field trip�
�	The attached Request For Administration of Medication Form 
must be completed by both the parent and the child�s physician and 
returned to the nurse as soon as possible, but by no later than 
March 1, 2010.  (Please make additional copies if needed so 
there is a separate form for each medication.)
�	Only send the medication(s) that it is absolutely necessary 
for your child to take while on the trip. Please do not send 
vitamins or herbal supplements.
1.	The medication(s) to be dispensed must be in the original 
over the counter packaging or pharmacy labeled container. Please ask 
your pharmacist to provide you with a small diameter labeled 
container for all prescription medications.
2.	Send the exact number of pills/doses your child will need 
while on the trip unless you would prefer the nurse to have a spare 
dose on hand.
3.	For once a day medications, please indicate on the orders if 
the dose is to be given in the morning or evening
4.	Medication(s) should be brought to the nurse�s office, by 
the parent, as soon as possible, but by no later than March 30, 2009 
for sorting and packing. 
�	All prescription and over the counter medications ordered by 
your child�s private physician will be held and dispensed by the 
nurse on the trip.  A student may not be in possession of any 
medication during the trip. 
�	An asthma inhaler or Epi-pen may be kept in the possession 
of the student and self administered provided orders from your 
private physician and written parental permission are on file in the 
nurse�s office.  One dose of Benadryl can be self administered only 
if it is written in conjunction with an Epi Pen order as part of a 
treatment protocol for anaphylaxis. These medications may not be 
shared with others.
�	Tylenol, Advil, Benadryl and Alka Seltzer Gold will be 
provided by the district and may be dispensed on the trip at 
the discretion of the nurse(s) in accordance with the Health Office 
Collaborative Orders approved by the District Medical Director.  It 
is not necessary for your private physician to write separate orders 
for these medications. Your permission to give these medications is 
addressed in Section II of the permission form.  These medications 
will be provided by the district and cannot be self-administered by, 
or in the possession of, the student.  (See above for Benadryl 
exception.)
�	Last minute medications prescribed by your child�s physician 
after the March 1st deadline date will be accepted up until 
departure.  You must provide the doctor�s order for the medication, 
written parental permission to dispense, and the medication in its 
original container.  Everything should be given to the nurse as soon 
as possible.

The music department trip involves the dispensing of medication to a 
large number of students. In order to expedite the process and avoid 
delays getting to the performance venues, medications will be 
dispensed during the trip as follows:
�	It is your child�s responsibility to report to the nurse at 
the prescribed time(s) to receive his/her medication. It is nearly 
impossible for the nurse to individually seek out each child 
receiving medication.
�	If you feel your child will not remember to report to the 
nurse and you do not want the dose of medication to be missed, 
please contact the nurse prior to the trip so special arrangements 
can be made.
