New Students

Medical requirements for entrance

Every new student entering Scotch Plains-Fanwood High school must 
submit proof of the following:

1. Documentation of a physical examination completed by your private 
physician within the previous 365 days prior to admission date.

2. For High School aged students, documentation of immunizations for
Tetanus, Polio, Measles, Mump, Rubella (MMR), and Hepatitis B.  At 
least one dose of each of these immunizations must be received before
provisional admittance to school will be granted.

3. Documentation of a skin test for Tuberculosis screening if 
entering from a country NOT LISTED HERE:
