Asthma QuestionsASTHMA ACTION PLANS FOR SCHOOL
If my child has asthma, how do I let the school know and what do I do? If your child has asthma be sure to notify the school nurse by writing a note or document it on your child's emergency form. What if my child needs meds while at school? Be sure to fill out the ASTHMA ACTION PLAN for your child, that can be found on this site under ASTHMA ACTION PLAN icon.Have your doctor fill it out, sign it and return to school. Do I send in the nebulizer for my child's treatments? No, each school has a nebulizer. Send in the medications, tubing, mask/mouthpiece along with the necessary meds. Be sure your child's name is on the bag. It will be kept at school for current treatments as well as future or emergency treatments. Be sure your doctor has signed the ASTHMA ACTION PLAN before you send in the medications. What if my child only needs an inhaler? Inhalers should be sent to school in the original prescription box. Be sure the ASTHMA ACTION PLAN states when the med. should be given. All medications need a doctor's signature for the nurse to give it during school hours. This signature is a part of the ASTHMA ACTION PLAN. What if my child only needs asthma meds for emergencies? If your child needs emergency asthma meds, they should be sent to school following the same protocol listed in question #4. An ASTHMA ACTION PLAN must accompany the emergency medications. If med is needed, you will be contacted by phone to inform you about the emergency or need for medication. Can I get the Asthma Action Plan from the nurse's web page? Sure! Just click on the icon for ASTHMA ACTION PLAN. You can print it out from your home/work computer and take it to your doctor for a signature. Be sure to fill out the top section where it asks for parent information. Does the Primary Nurse have a fax number for this plan to be sent? Yes, my fax number is: 390-4763. Let your physician know it goes directly to my office (does NOT go through the front office). Can my doctor get access to your web page to print the Action Plan? Yes! Ask your doctor to go to: http://teacherweb.com/NJ/UpperTownshipPrimarySchool/Hallsworth/ Click on ASTHMA ACTION PLAN. If my child needs an inhaler with a chamber, does it have to come to school? Chambers are the BEST way for a child to receive inhaler medications. Chambers ARE expensive if purchased locally! I recommend you check the School Health Web Site at: www.schoolhealth.com (Catalog # 61675) These spacers are only $7.40 on line! Buy one to keep @ school. This item is called a Pocket Chamber on this website. |
