KINDERGARTEN REGISTRATION (School Year 2010-2011)

YOUR CHILD MUST BE 5 YEARS OLD BY OCTOBER 1, 2010

In order to register you must have the following items at the time of registration. You will not
be permitted to register if ANY of these items are missing. (You will be asked to return to
school when all items are presented):

· Birth Certificate/Baptismal Certificate
· 2 Proofs of Residency (Driver’s License, Utility bill, Tax bill, etc;)
· Proof of Immunization Record (Shot records from doctor’s office)
(These records may NOT have the kindergarten boosters on them. All boosters and physical must be obtained BEFORE kindergarten starts in September (9/2010).

Parents of children that are currently enrolled in the Primary School's Pre-School Program for
the 2009-2010 school year, WILL NOT NEED TO ATTEND REGISTRATION. Information concerning immunization updates will be sent to you through your child's classroom teacher. If you do not receive a notice by April 2010, please call me or email me at: hallsworth@upperschools.org
