Forms for children with Special Medical needs (diabetes, siezure disorder, asthma, anaphylactic food allergy) can be found on the district website under forms and then Nurse. They are also available on this website under the forms sections. These forms are required for your child/children to be able to receive medications and appropriate care during the school year. All forms must be completed by your physician and sent directly to the school nurse.
If your child needs to take medication during the school day please read the instructions under the Medication section of this website and fill out the appropriate forms. (even medications that are over the counter).
Anytime your child has a physical examination or immunizations given in the doctors office please remember to send in the completed forms and documentation. This allows us to keep a complete and up to date health record for your child/children.
