Complete this Graded Response Sheet to demonstrate your knowledge of
the Procedures and Regulations we expect students to follow to be
successful in this class. The words to use for the short answer
section are included on the hand out. Use only the words that apply.
More choices may be provided than are needed.

  • 1) Shirts that allow freedom of movement, cover the clavicle, do not
    show cleavage and cover the midriff are required activity attire.

  • 2) Sports shorts, sweatpants or windpants not jeans, jean shorts,
    pajama bottoms, school sports team wear or cargo pants are required
    activity attire for class.

  • 3) Students who play on a sports team may use the Team Locker Room to
    store their clothes for Physical Education class.

  • 4) Headgear, which includes any type of hat, can only be worn in class
    if it is safety gear that the teacher has approved.

  • 5) If a student is late to class the student may lose participation
    points which will lower that student's grade.

  • 6) Constant tardiness may result in no credit for the class.
  • 7) Students must participate in all of the class activities
    satisfactorily to receive full credit for class.

  • 8) Physical Education Class is a laboratory class which means you must
    be in class and actively participating in class to earn credit for
    class unless you have a medical excuse, which then means you must
    make up the activities in some way.

  • 9) If a student has failed the course, or wishes to have a better
    grade in the class, the student has 5 weeks into the new quarter
    to improve the grade by making up classes that have been missed.

  • 10) Physical Education is a semester course, so once the semester is
    over a student may not make up any work that is missed to improve
    his/her grade.

  • 11) If you fail the semester you will have to make up the class in
    Summer School or take the class again in your Senior Year to get
    enough credits to graduate.

  • 12) Jewelry can be worn in class because it is unsafe to leave it in
    the locker room.

  • 13) The only type of shoes that can be worn for class activities are
    sneakers that fasten and have the back on them so that they stay on
    your feet and are safe.
