Sports Physicals

Any student who wishes to participate in interscholastic sports this fall must have a sports-oriented physical examination and evaluation; this exam is valid for one year. A student’s initial physical for sports must be performed by the school examiner. Any subsequent sports physical done outside the school must receive prior authorization from the Health Office. To arrange a sports physical in school: 1. Students should pick up the Athletic Health History form at the Health Office 2. This needs to be completed and signed by a parent/guardian. 3. Return the form(s) to the Health Office by May 1st. Sports physicals are now being scheduled in the school health office through May. If your child wishes to have a physical, it must be completed this spring. There will be NO PHYSICALS GIVEN during SUMMER VACATION.
