Spring Sports Physical
Any student who wishes to participate in interscholastic sports this spring
must have a sports-oriented physical examination and evaluation; this exam is
vaild for one year. A stdent's initial physical for a sport must be
performed
by the school examiner. Any subsequent sports physical done outside the
school must receive prior authorization from the Health Office. In order to
have a sports-oriented physical the student must hand in a Athletic Health
History Form.
Spring Sports are:
Girls: Softball
Soccer
Track & Fild
Boys: Baseball
Track & Field
2. Studetns should pick up an Athletic Health History form at the Health
Office or the Gym. For you convenience, the Athletic Health Form may also be
downloaded from this site.
3. The form needs to be completed and signed by a parent/guardian.
4. Please return the form to the Health Office by Friday, February 6th.
