All students who wish to particpate in Hoops for Heart must hand in their 
fund raising packets to their P.E. teacher by Friday 4/27.  Hoops for heart 
will be held during P.E. classes on Monday 4/30 and Tuesday 5/1.

All physical education class must complete the online badminton quiz 
by 5/4/12.

SPRING SPORTS & HEALTH APPRAISAL REQUIREMENTS

Boys:     Baseball
             Track

Girls:     Soccer
             Softball
             Track

Any student who wishes to participate in any of the interscholastic sports 
above must have a sports-oriented physical examination and evaluation; this 
exam is valid for one year. A student’s initial physical for sports must be 
performed by the school examiner. Any subsequent sports physical done outside 
the school must receive prior authorization from the Health Office.

Physicals for spring sports are scheduled to begin the week of February 
6th. Students may check the lists posted throughout the school to determine 
their need for a sports physical. To arrange a sports physical in school:

1) Students should pick up the Athletic Health History form at the Health 
Office; it is available online at,
