SUMMER SPORT PHYSICAL DATES: ALL TUESDAYS, JULY: 10, 17, 24, 31, AUGUST: 7, & 14. ATHLETES MUST BE PRESCREENED BY THEIR SCHOOL NURSE PRIOR TO SCHEDULING A PHYSICAL. ATHLETIC PHYSICAL PROCESS Students who wish to participate in interscholastic athletics need to have the following information: I. Athletic Physicals: This can be performed free of charge in the nurse’s office on specific dates during the summer and certain dates prior to the start of an athletic season. These dates have been posted on the athletic web page and CHS Nurses page. It can also be done through your private physician. The processes are outlined below. A. Free School Physician Exam: The dates for the sports physicals HAVE BEEN posted and announced in the school buildings. They will also be posted on the school nurse’s web page as well as the athletics web page. 1. Your child must make an appointment with the nurse in the building prior to the physical date. If it is to be done during the summer, you must call to make an appointment ONLY AFTER YOUR CHILD HAS BEEN PRESCREENED BY HIS/HER BUILDINGS NURSE. 2. You must fill out the Athletic Health History form and return it to the building nurse. Once we have both forms the school nurse will issue a verification card which will be given to the Coach, not the athlete. 3. The Athletic Health History form can be found on the nurse’s web page and the athletic web page. B. Private Physician Physical: The physical must be on the approved CCSD physical form. It can be found on the nurse’s web page and the athletic web page along with the Athletic Health History form. 1. The physical must be completed, signed and stamped by the physician. It should then be brought to the building nurse one month prior to the start of the specific sport season. The district then makes arrangements for the school physician to sign off on it. BEING GOVERNED BY THE NEW YORK STATE PUBLIC HIGH SCHOOL AHTLETIC ASSOCIATION AND BY THE REGULATION OF THE COMMISIONER OF EDUCATION “A STUDENT WHO MAY ENGAGE IN INTERSCHOOL COMPETITION SHALL RECEIVE AN ADEQUATE HEALTH EXAMINATION AND HEALTH HISTORY UPDATE WHEN REQUIRED, AND MAY NOT PARTICIPATE WITHOUT THE APPROVAL OF THE SCHOOL MEDICAL OFFICER.” 2. You must also fill out the Athletic Health History form and have your child bring it to the building nurse. 3. Once we have the approved physical and the health history form the nurse will issue a verification card, which will be given to the coach, not the athlete. II. EXPIRATION DATES: The physical itself is good for one full year and will expire on the last day of the month in which it was performed. For example, a physical that was performed on May 5, 2011 will expire on May 31, 2012. The health history form must be filled out by you prior to the start of each new season. This means that if your child wants to try out for a fall, winter and spring sport you will need to fill out the form each time.
