NOTICE TO PARENTS
ALL SPORTS PAPERWORK MUST BE IN TO THE HEALTH OFFICE BEFORE THE STUDENT ATHLETE MAY PRACTICE.--THIS INCLUDES: PHYSICAL ( with in a year), HEALTH HISTORY and MEDICAL RELEASEEACH SEASONMore information for FALL SPORTS will be forth coming.All students who wish to participate in interscholastic athletics must obtain FORM REVIEW approval by the school nurse before they may begin practice. Prior to each season, the student must sign up for their desired sport in the Health Office. Fall signups in MayWinter signups in OctoberSpring signups in FebruaryThe student will be notified at that time if a physical exam is needed. Your family physician or the school health staff may do annual sports physicals. You are encouraged to have your family physician conduct your child’s sports physical at the time of their annual check-up. Your family physician knows your child’s health history, and can best evaluate their fitness for sports. If you choose to have a private physical examination, it must be completed on the school district’s physical examination form.If your child has a health problem such as asthma, diabetes, an orthopedic condition, seizure disorders, allergy to insect bites, etc., please have your family physician note the condition and any instructions relative to sports participation. Any Medications can be ordered on the school health form as well, with the ability to indicate if the student-athlete is self directed.Any student athlete who is removed from play by a physician, can only be reinstated to play by that physician or a specialist in that field.
